Adaptasi AHRQ-Hospital Survey on Patient Safety (version 2.0) kedalam bahasa Indonesia

Introduction: Measurement of safety culture was a form of commitment from hospital leadership whose results were used to improve the implementation of the hospital's quality improvement and patient safety program. AHRQ in 2019 has released a hospital survey on patient safety culture version 2.0 which has good reliability and has been widely used in various countries. This study aims to adapt the survey instrument on patient safety culture 2.0 into Indonesian.
Methods: This research was an observational study, the researcher has obtained permission from AHRQ to translate it into Indonesian. The research was conducted at Syamsudin Hospital. 676 subjects participated. Content validity was carried out on 3 experts, reliability test using Cronbach alpha was carried out on 10 dimensions of composite mesuare and compared with the English version.
Results: content validity index showed expert agreement with a value of 0.92. All question items have good validity. Reliability test results on the composite measure of supervisor,manager or clinical leader support for patient safety 0.75, teamwork 0.79, communication oppeness 0.75, reporting patient safety events 0.79, organizational learning 0.72, communication about error 0.87, hospital manajemen support 0.72, response to error 0.75, handsoff and information exchange 0.72 and staffing and work pace 0.84. there is no difference in Cronbach alpha values for each composite measure between the Indonesian and English versions.
Conclusion: The Indonesian version of the AHRQ Hospital Survei on Patient Safety Culture version 2.0 is reliable so that it can be used to measure patient safety culture in Indonesian hospitals
